Appointments to begin at 7:30 a.m. Tuesday, Feb. 23
Mesquite’s Operation Vaccination task force announced today that appointments for COVID-19 virus vaccine shots will open to those who are 65 and older. Anyone age 70 and older will still be able to make appointments locally.
The website to schedule appointments online will open up at 7:30 a.m. Tuesday, Feb. 23 and will have slots for six days. The website address is https://health.mesquitenv.gov/home. In the lower screen is a graphic labeled Vaccinations that will take you to the appointment page.
Vaccinations will be available for three days this week: Wednesday, Feb. 24, Thursday, Feb. 25 and Friday, Feb. 26 between 7:30 a.m. and 12:30 p.m. In addition, appointments will be available tomorrow for Wednesday, Mar. 3, Thursday, Mar. 4 and Friday, Mar. 5 between 7:30 a.m. and 12:30 p.m.
If you try to access the website before the appointment system is activated on Tuesday mornings or after all appointments are fully booked, you will see a screen like the one below that asks for a password. That is only a placeholder. Once the system is live and taking appointments, you will not need a password.
After many people could not get appointments by phone or obtain one in person at City Hall because the appointments went so fast online, the task force has set aside a number of appointments for offline booking. People can call 702-346-8446 or visit City Hall from 7:30 a.m. to 5:00 p.m. Monday through Thursday or Friday, 7:30 a.m. to 11:30 a.m. while appointments last.
Those people who received their first dose of the vaccine the first week of February have already received an appointment for their second dose beginning Mar. 3. Those appointments will not show in the scheduler as they have already been given out.
People residing in Mesquite, Bunkerville, and the Arizona Corridor of Scenic, Beaver Dam, and Littlefield are eligible to sign up for the vaccinations provided they are in the appropriate age group including those who reside in these areas part-time. There will be assistance available for the physically disabled to move from their vehicle into and out of the Rising Star Fieldhouse.
Everyone must complete the two release/information forms that are available through the link below or are available as they make their appointment online. For those who cannot print them or access them online, the forms are available at Mesa View Regional Hospital or city hall.

There is a new version of the second form that everyone must complete: It is available through these links:

The vaccination location is the Rising Star Field House graciously provided by the Eureka Casino. The Field House is located at 333 N. Sandhill Blvd behind the hotel.
Do not arrive at the Field House more than 15 minutes before your scheduled appointment but at least 10 minutes before you are scheduled.
Have all forms completed before you arrive at the vaccination site. Do not wait until you arrive for your appointment to fill out the forms.
Bring your identification card.
Wear appropriate clothing that allows for easy access to your upper arm.
Vaccinations will be offered at no cost.
Don’t try to game the system. If you are not eligible for the vaccination at this stage, don’t try to sneak in. You will have to show identification at the vaccination site and if you are not 65 years old or older, or otherwise eligible, you will be denied a vaccination. The appointment website will also have you verify your age at several steps in the process. Each week the State and Health District will evaluate the possibility of opening the next category of vaccine recipients.
If you have underlying health conditions contact your personal health care professional to answer questions you may have about the safety of receiving the vaccination. Personal health care advice will not be available at the vaccination site.
The technology supporting the website registration and appointments does not allow for waiting lists at the present time. It can only be used to book an appointment. The task force clearly expects glitches and heavy traffic on the website in the early stages of the roll-out. Be patient and know that the task force is working very hard to make the entire process as easy as possible.
Appointments for vaccinations will be determined by the number of vaccine dosages actually received and on-hand. The task force does not want to overbook appointments only to have to cancel them later.
People will receive the Moderna vaccine. Click here for a fact sheet related to the Moderna vaccine that you can review prior to your appointment: MODERNA_eua-fact-sheet-recipients
A health care professional will administer the vaccine. After receiving the vaccine, you will proceed to a waiting area for 15 – 30 minutes as a precautionary measure to monitor for any reactions to the vaccine. While adverse reactions are rare, this waiting period is required for everyone who receives the vaccine.

Do those of us that have had the first vaccine have to fill out additional forms since we already turned in forms when we got our first shot? If so, do we go to the links in this article? Thank you for all the great information you have given us during this process.
Mesquite Citizen Journal says
Carolyn,
No, you don’t have to fill out additional forms. The forms you turned in for your first shot will cover your second shot.

The answer to your question is in the article. It’s in the paragraph that begins with “If you try to access the website before the appointment system is activated on Tuesday…” and includes a picture.
